New patients should plan to arrive early enough to complete registration. Please bring the items relevant to your visit:
- Driver's license or state ID
- Referral, if required by your insurer
- Insurance information
- Prior operative records, medical records, X-rays, MRIs or CT scans
- A current medication list and known allergies
